Hospice donation information

Early in the development of the UC Davis Hospice Program, a memorial fund was established in order to provide an opportunity for friends and relatives to honor the memory of loved ones.

Donations are used to provide end-of-life care to terminally ill patients who otherwise would not receive care and to assist with other special patient needs. Funds are also utilized for staff and volunteer training, as well as education.

Donations to the Hospice Memorial Fund may be made via check, money order or major credit card payable to the UC Regents. Please indicate that the donation is for the Hospice Memorial Fund and note in whose memory or honor the contribution is being made. You will receive acknowledgement of your donation and the family or caregiver will be informed of your memorial gift. If you would like a particular person to receive notice of your donation, please let us know. Contributions may be sent to:

UC Davis Home Care Services
Hospice Program
3630 Business Drive
Sacramento, CA 95820

You may also make a donation online via our secure online giving form.  Please choose Hospice from the drop down recipient menu.  http://www.ucdmc.ucdavis.edu/giving/donations/online.html
